• $175 per Intake assessment
• $150 for 50 minute individual or family session
• $350-$450 DOT-SAP Assessment

Insurance

I am in network with Cigna, Horizon BCBS Commercial, Aetna, and United Healthcare plans. I remain out-of-network with other plans. I also accept all major credit or debit cards for payment.

Uninsured NJ patients with gambling disorder may qualify for 800 Gambler Grants.

Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services.

I’d recommend asking these questions to your insurance provider to help determine your benefits:

• Does my health insurance plan include mental health benefits?
• Do I have a deductible? If so, what is it and have I met it yet?
• Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
• Do I need written approval from my primary care physician in order for services to be covered?

SAP Rates

SAP Services are available for the entire state of New Jersey using a HIPAA compliant virtual platform by appointment only.

Standard SAP services are $350-$450, payable during your first visit. The fee includes the initial SAP evaluation, referral services to appropriate education or treatment providers, progress, and compliance monitoring, reporting to your DER (Designated Employer Representative) or the FMSCA Clearinghouse if applicable, any follow-up SAP evaluation, and record-keeping.

Additional fees may apply for non-standard services and for any third party laboratory testing.

PLEASE NOTE: the above fee does not cover provider fees for the recommended education and/or treatment. I will consider your insurance coverage for placement; however, you are ultimately responsible for any costs associated with the recommended treatment.

Payment

We accept cash, checks, and all major credit cards as forms of payment.

Cancellation Policy

If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged for the full rate of the session.
